To expedite the booking process, we request that all prospective patients fill out a Consultation Intake Form. It helps us to understand your goals, and to streamline the consultation process. Once you have submitted your form, a patient coordinator will be in touch with you to book your consultation.
Your information will be securely stored in our Electronic Medical Records system, and it will never be shared.
Intake form is valid for 12 months once submitted. Should your consultation take place more than 1 year after submitting this form, we will require you to re-submit.
